About Grace Sok, NP

Grace Sok is a family nurse practitioner dedicated to providing compassionate, patient-centered care. She earned her degree from Chamberlain University and brings a strong clinical foundation to family practice. Grace has four years of experience as a registered nurse on a medical-surgical unit, where she developed expertise in patient assessment, education, and care coordination. She was recognized with a DAISY Award for Extraordinary Nurses, with one patient sharing that her presence brought “sunshine on a cloudy day.”


In her role as a nurse practitioner, Grace focuses on building meaningful relationships with her patients while delivering safe, evidence-based care. She has a strong interest in chronic disease management, patient advocacy, and comprehensive care. She was drawn to healthcare by a desire to support patients during vulnerable moments and believes effective care starts with truly listening. Her philosophy centers on providing compassionate, individualized care that respects each patient’s unique story. Outside of work, Grace enjoys spending time outdoors – especially near the water – as well as golfing and staying active with family and friends.
